Documents You Need for GP Registration in Sheffield

 When registering with a GP Sheffield, many people wonder what documents are needed. At Beauchief Medical Practice, we’ve simplified this process.

Here’s what you’ll typically need:

  • Proof of address (e.g., utility bill, rental agreement)

  • Photo ID (e.g., passport or driving licence)

If you’re registering online, you can simply upload scanned or clear photos of these documents directly to the form.

For students, a university letter showing your address is also acceptable. For children, a parent or guardian’s documents may be used.

While having your NHS number is useful, it’s not required. If you don't know it, you can find it online.

Once registered, you’ll be assigned a Named GP and gain access to our full range of healthcare services, including check-ups, blood tests, prescriptions, and long-term condition management.
