Here’s what you’ll typically need:

Photo ID (e.g., passport or driving licence)
If you’re registering online, you can simply upload scanned or clear photos of these documents directly to the form.
For students, a university letter showing your address is also acceptable. For children, a parent or guardian’s documents may be used.
While having your NHS number is useful, it’s not required. If you don't know it, you can find it online.
